Part of the Proof of packing guide
Free evidence templates for packing, receiving, 3PL contracts and claims

On this page
Most disputes are lost before anyone opens a claim form. The damaged carton was moved before anyone photographed it, the count was written on a scrap of paper, or nobody can say which packer sealed which box. A good template fixes that. It makes the right step the obvious one, in the right order, every time.
The templates below are built around one rule from our proof of packing guide: a record only becomes evidence when it is tied to a specific reference and can be checked by someone who doesn't trust you. So every template asks for the reference first and points each observation to a photo or a recording.
Packing and order templates
These templates cover the packing bench, where most refund, return and chargeback disputes are decided.
| Template | Use it when | Guide |
|---|---|---|
| Packing station SOP | Setting up or auditing a bench, training new packers | Packing station SOP template |
| Packing video checklist | Checking that each recording shows what a reviewer needs | Packing video evidence checklist |
| Subscription box QC checklist | Kitting and monthly box runs, box-by-box quality checks | Subscription box QC checklist |
The packing station SOP is the one to print first. It sets out the one-take sequence that holds up in a dispute: the order barcode scan, the item and its condition, the shipping label, the padding going in and the box closing. That sequence is the same one buyers, banks and carriers look for when they review seller dispute evidence by platform. Carriers ask the same question in their own words: UPS lists the supporting documents it wants with a damage claim, and payment processors such as Stripe want the evidence as documents and images, not video.
Receiving and warehouse templates
These templates cover what happens before an order exists: inbound freight, discrepancies and damaged stock.
| Template | Use it when | Guide |
|---|---|---|
| Receiving damage record | A delivery arrives with visible damage | Freight damage at warehouse receiving |
| Receiving discrepancy (OS&D) report | Counts are over, short or damaged against the PO or ASN | Receiving discrepancy report template |
| Inbound container inspection checklist | Unloading containers and trailers, checking seals | Inbound container inspection checklist |
| Damaged inventory quarantine SOP | Stock is found damaged in storage, on a pick or at a count | Damaged inventory quarantine SOP |
Together these give a warehouse a complete trail from the dock door to the shelf. When a brand, a supplier or a carrier later asks what happened to a unit, the answer is a reference number and a set of photos, not a memory. For freight, the US claim rules in 49 CFR Part 370 expect a written claim that identifies the shipment and the amount, which is exactly what a reference-first record makes easy. Our 3PL evidence playbook explains how the trail continues through pick-pack, loadout and returns.
3PL contract and transition templates
These templates cover the commercial side of fulfillment: what a 3PL and a brand agree to treat as proof, and how stock moves between providers.
| Template | Use it when | Guide |
|---|---|---|
| 3PL contract evidence clause | Drafting or renewing a fulfillment agreement or SLA | 3PL contract evidence clause |
| 3PL-to-3PL transfer checklist | Moving inventory from one fulfillment provider to another | Switching 3PLs: inventory transfer checklist |
The contract clause is the cheapest dispute you will ever settle, because it settles the argument about evidence before any argument about a specific order. It defines what counts as substantiation for a mispick, a shortage or a retailer chargeback flowed down to the 3PL. Legal commentators such as Terms.Law and contract guides such as Cleverence cover the commercial clauses; the template adds the evidence standard. Pair it with the guides on 3PL mispick disputes and retail compliance chargebacks.
High-value shipping templates
These templates cover goods where a single claim can cost more than a month of margin.
| Template | Use it when | Guide |
|---|---|---|
| Artwork condition report | Shipping paintings, prints and framed work | Artwork condition report before shipping |
| Jewelry shipping security SOP | Shipping fine jewelry, diamonds and watches | Jewelry shipping security SOP |
For these goods the strongest step is not on paper at all: it is letting the buyer see and approve the exact piece before it ships. The photo approval before shipping guide explains how that works.
How to turn a template into evidence
A completed template is a summary. Evidence is what each line points to. Three habits turn one into the other:
The third habit is where most paper processes break down, and it is the reason we built PallasMark the way we did.
How PallasMark digitizes these templates
Every template here maps onto a PallasMark record you create on the phones your team already carries. The reference on the form becomes the order, shipment or pallet the record is tied to: scan the barcode and the right record opens. The checklist steps become the photos and the one-take recording. Values like seal numbers, PO numbers or serial numbers go into custom fields, and on the Scale plan the app can read them straight off a label photo with AI label reading.
Each file is fingerprinted when it is stored, and anyone you share it with can check it on a public verification page without an account. A warehouse can share one specific record with a client brand, carrier or supplier through a link with an expiry date and an optional PIN, without opening the rest of the warehouse. When a dispute needs a document, the record becomes an evidence PDF with stills from every video.

Frequently asked questions
Are these templates really free to use?
Yes. Every template is a free PDF with no sign-up and no email gate. You can print it, copy it into your own documents and adapt the wording to your operation, your carriers and your contracts.
Which template should a 3PL start with?
Start with the receiving discrepancy report and the damaged inventory quarantine SOP, because most brand disputes trace back to what was counted and what condition stock was in when it arrived. Add the contract evidence clause at your next client renewal so the evidence standard is agreed before the next dispute.
Is a filled-in paper form enough evidence on its own?
It is a good start, but a form is still your own statement. It becomes much stronger when every line points to a photo or video tied to the same reference, such as the PO, pallet or order number, and stored so that any later edit shows. That is the step PallasMark adds.
Can I use these templates with PallasMark?
Yes. Each template maps to a PallasMark record: the reference becomes the order, shipment or pallet the record is tied to, the checklist steps become the photos and the recording, and custom fields hold values such as seal numbers or PO numbers. The record is fingerprinted and can be shared with a client or carrier through a link.
Do you add new templates?
Yes. This page lists every template published on the PallasMark blog, and new ones appear here as they are added.
